SIGN UP AND ACTIVATE YOUR FAMILY PORTAL:
Please do not request appointments through the portal or send pictures without speaking to a doctor first.
The patient portal is called “My Kids Chart.” There are a few ways you can sign up:

- Call 203-972-4250
- Sign up at the front desk at your next visit.
You will receive an email and/or text with a temporary password to the email or cell phone associated to the account/child (there can be more than one email associated to the account/child). Please note, it may take up to 24-48 hours to receive if requesting on a weekend.
If you are already registered:
Click on the link to My Kids Chart and enter your password.
If you are having trouble linking to My Kids Chart, copy and paste the following into your browser: cpp.pcc.com/portal/session/new

IS YOUR CHILD 18 YEARS OR OLDER?
If your child is 18 years or older, they have to grant permission to the parent/guardian to access their medical records. If you wish to access your young adult’s records/portal, please have him or her fill out the following form by clicking here.
Please allow 24 hours for permission to be activated.
INFORMATION YOU CAN ACCESS:
When you access our patient portal, you can review a complete health information summary for each child in your care including:
- Most recent physical date and upcoming appointments
- Historical visits
- A summary of labs and medical tests
- A problem list, allergy list and medications list
- A complete immunization record (you print directly from the portal)
HOW TO SEND A MESSAGE THROUGH YOUR PATIENT PORTAL:
Once you are activated, you can send a message to anyone in our office through your portal. We recommend you put the department and/or person you are trying to reach in the subject line of the message so that it is sent to where you want it to go.
Note that messages should not be used for urgent matters. Please do not send pictures unless you have been instructed to do so by your doctor at the time. The portal messages may take up to 24 hours for a response.
PAY YOUR BILL THROUGH THE PORTAL:
You can pay your bill through the portal at anytime. You will see your balance in the Personal Balance section, just click on the “Make a Payment” button and follow the prompts.
